At the bottom of this page you’ll find four form options to mail-in your donation. Each will open a PDF form you can print.
What you need to know:
- The PDF will open in your web browser and you should see a menu on the bottom right with options to save the form, print the form, etc.
- The PDF can only be printed while it is blank. If you fill in the form while in the browser, you will not be able to print the form.
- If you are unable to print the form, or if for any reason you don’t want to print the form, you can still mail your donation. Just include your information with a check made out to American Macular Degeneration Foundation and mail to:

If you would like to open the form and print to mail in with your donation, please select from the following:
Regular donation with option to receive AMDF’s printed newsletter if you donate $25 or more.
Regular donation with option to gift AMDF’s printed newsletter to someone other than yourselfif you donate $25 or more.
Memorial donation in memory of a friend, loved one, or relative who has died, with option to receive or gift AMDF’s printed newsletterif you donate $25 or more.
Special occasion donation to honor a special someone or date, with option to receive or gift AMDF’s printed newsletter if you donate $25 or more.
